Transitions of Care Pilot Poster Presentation
We are excited to tune this Thursday, October 14, as two of our teammates present their poster on a Transitions of Care project.
The project is comprehensive transitions of care pathway for patients undergoing surgery can help mitigate the financial and human cost of uncoordinated care with respect to opioid pain management.
This poster was submitted for Michigan Medicine’s Quality Month celebration. This celebration highlights important improvement work happening across Michigan Medicine.
Not only was this poster accepted initially, but our team was asked to return for a second presentation and live Q + A session as well.
