OPEN partners with Packard Health to provide clinical care services. If you would like to give permission for the OPEN Warmline team to share your information with other providers, please sign and complete this form.
The Warmline is required to ask patients if they will participate in a voluntary survey. Completion of the survey is optional, and the patient can still receive Warmline services if they decline. The survey should only be completed by a patient that has already been seen by the Warmline.
